We require 24 hours to complete all forms. There is a $10 charge for all forms unless the form is filled out during a well check. A pre-sports screening form must be filled out by a parent only before the doctor can complete any sports form.
SPORTS FORMS -- For PIAA forms, the patient must have a well check after June 1st. COMMUNITY SPORTS -- The patient's last well check must be within the past year. HEALTH APPRAISAL FORMS -- The patient's last well check must be within the past year. |
